Community Living Center- Registered Nurse

The Community Living Center Registered Nurse provides direct patient care to residents in a long-term care setting, focusing on promoting independence, dignity, and quality of life. Responsibilities include: Clinical Care: Delivers fundamental, knowledge-based nursing care to assigned residents, developing technical competencies in medication administration, wound care, and monitoring vital signs. Integrates biopsychosocial concepts to address both basic and complex health needs. Patient Assessment: Conducts comprehensive assessments of residents' physical, emotional, and psychosocial status, identifying changes in condition and initiating appropriate interventions. Care Planning: Collaborates with interdisciplinary teams to develop, implement, and evaluate individualized care plans that align with residents' goals and preferences. Patient Advocacy: Serves as a primary advocate for residents, ensuring their rights, preferences, and cultural values are respected in all aspects of care. Education and Support: Provides health education to residents and families, promoting self-care and disease management. Offers emotional support and guidance during transitions or end-of-life care. Leadership and Collaboration: Leads or participates in team meetings, quality improvement initiatives, and safety rounds. Mentors new staff and nursing students as needed. Documentation: Maintains accurate, timely, and confidential electronic health records in compliance with regulatory standards. Evidence-Based Practice: Applies current research and best practices to improve care outcomes, participating in unit-based projects or committees. Emergency Response: Responds to medical emergencies within the facility, providing life-saving interventions as trained.

Qualifications and requirements include: - Valid Registered Nurse (RN) license in the state of practice. - Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with commitment to obtain BSN within specified timeframe. - Minimum of 1 year of nursing experience, preferably in long-term care, geriatrics, or community living settings. - Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred. - Strong clinical assessment and critical thinking skills. - Ability to work effectively in interdisciplinary teams. - Excellent communication and interpersonal skills. - Proficiency in electronic health record systems. - Physical ability to perform tasks such as lifting, transferring, and standing for extended periods. - Flexibility to work rotating shifts, weekends, and holidays.
