Registered Nurse Navigator-Care Coordinator

The Registered Nurse Navigator-Care Coordinator is responsible for providing competent, evidence-based care to assigned patients. This role involves patient management, care coordination, and discharge/disposition planning for both inpatient and outpatient settings. The Case Management Registered Nurse assists Veterans, family members, and caregivers in receiving the most appropriate options and services to meet their complex health care needs.
