Nurse PACT

The Nurse PACT (Patient Aligned Care Team) is responsible for delivering comprehensive, patient-centered nursing care within a primary care setting. Key responsibilities include: Direct Patient Care: Provide fundamental to complex nursing care based on patient needs, utilizing biopsychosocial concepts and evidence-based practices. Assess, plan, implement, and evaluate care plans for assigned patients, ensuring holistic and coordinated care.

Basic Requirements: U.S. citizenship. Proficiency in written and spoken English (per 38 U.S.C. 7403(f)). Graduation from a school of professional nursing approved by the appropriate accrediting agency (e.g., ACEN or CCNE). Foreign nursing education requires equivalency validation (e.g., from ICD).
