Nurse HBPC

The Nurse HBPC (Home Based Primary Care) provides direct patient care to veterans in their homes, coordinating with interdisciplinary teams to manage chronic conditions and promote health. Responsibilities include conducting comprehensive nursing assessments, developing and implementing care plans, administering medications and treatments, educating patients and families on disease management, and monitoring patient progress.

Basic Requirements: U.S. citizenship; proficiency in written and spoken English; graduate of a school of professional nursing approved by ACEN or CCNE (or equivalent for foreign graduates); current, full, active, and unrestricted registration as a graduate professional nurse in a U.S.
