The Registered Nurse (RN) Care Coordination and Integrated Case Management (CC&ICM) Initiative Coordinator is administratively responsible for the clinical programming and daily operations of the Clement J. Zablocki VA Medical Center (CVAMC) CC&ICM initiative. This role carries both clinical and administrative responsibility to ensure the effectiveness of services, application of the latest VHA guidelines, and modification of service patterns as needed.

Basic Requirements: English Language Proficiency (per 38 U.S.C. 7403(f)). Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following at the time the program was completed: Accreditation Commission for Education in Nursing (ACEN)
