The Women's Health RN Care Coordinator is a member of the Patient Aligned Care Team (PACT) Core Team, which includes the Patient, Primary Care Provider, and Clinical Care Associate, as well as the Expanded Team comprising family, caregiver, and internal and community-based resources. The role involves providing veteran care and developing patient-centered care plans. The RN collaborates with internal and external VA services to facilitate care transitions and effectively meet the needs of Women Veteran patients.

Basic Requirements: English Language Proficiency as per 38 U.S.C. 7403(f). Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE at the time of program completion.
