The Referral Coordination Team Registered Nurse is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.

The incumbent is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.

Key responsibilities include managing referrals, coordinating care across departments, ensuring timely access to services, and maintaining accurate documentation.

The nurse will collaborate with interdisciplinary teams to facilitate patient transitions and optimize outcomes.

Qualifications: Must be proficient in basic written and spoken English.
