岗位职责
As a Social Worker with the Outpatient Patient Aligned Care Team (PACT) within the 32nd St. Clinic, you will support Social Work Service with the Phoenix VA Healthcare System. Your responsibilities include:
Independently implementing treatment modalities to address the needs of Veterans and their families.
Identifying the level of functioning and degree of impairment in occupational, social, academic, interpersonal, and family domains.
Evaluating and assessing lethality risk and performing mental status exams (MSE).
