Care Coordination: Facilitate interdisciplinary collaboration to ensure seamless transitions of care, including admission, discharge, and follow-up planning. Develop and implement individualized care plans based on patient needs, preferences, and clinical guidelines.
Utilization Management: Review patient records to assess medical necessity, appropriateness of care, and resource utilization.
