RN - Community Care Coordinator

The Registered Nurse (RN) Community Care Coordinator serves as a member of the Geriatrics and Extended Care (GEC) Community Care Team. The primary responsibility is to execute a streamlined approach to receiving, triaging, and directing timely, coordinated care for veterans. This role involves coordinating skilled nursing and home infusion services within the community care network, ensuring seamless transitions and continuity of care. The RN will assess patient needs, develop care plans, and collaborate with interdisciplinary teams to optimize patient outcomes.

Must be proficient in basic written and spoken English. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
