Social Worker-Suicide Prevention Case Manager

The Suicide Prevention Case Manager (Social Worker) is responsible for providing comprehensive case management services to Veterans at risk for suicide. Key responsibilities include conducting suicide risk assessments, developing and implementing safety plans, and coordinating care with interdisciplinary teams. The role involves direct clinical intervention, including crisis intervention and evidence-based therapies, to address suicidal ideation and behaviors. The case manager will collaborate with community partners, such as local mental health agencies and emergency services, to ensure continuity of care. They will also participate in outreach activities to identify and engage at-risk Veterans, provide education on suicide prevention, and maintain accurate clinical documentation in accordance with VA policies. Additionally, the position requires monitoring patient progress, adjusting treatment plans as needed, and participating in quality improvement initiatives to enhance suicide prevention efforts. The case manager will serve as a resource for other staff on suicide prevention best practices and may provide training to clinical teams. They will also be involved in data collection and reporting to track outcomes and identify trends in suicide risk among the Veteran population.

Qualifications include a master's degree in social work from a school fully accredited by the Council on Social Work Education (CSWE). Candidates must be licensed or certified by a state to independently practice social work at the master's degree level, with licensure required for GS-11 grade level and above.
