Registered Nurse - Community Care Coordinator

The Community Care Coordinator, RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. This role assesses, plans, implements, and evaluates the effectiveness of the Veteran's care. The coordinator serves to support the delivery of high quality, timely care to the Veteran receiving services in the community. Key responsibilities include coordinating care across community providers, ensuring continuity and quality of care, and acting as a liaison between the Veteran, their family, and community healthcare partners.

Basic Requirements: English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
