RN - Geriatrics and Extended Care Community Care Skilled and Home Infusion Coordinator

The Geriatrics and Extended Care (GEC) Community Care Skilled and Home Infusion Coordinator Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care for patients within the GEC Community Care program. This role involves coordinating skilled nursing and home infusion services, ensuring seamless transitions between care settings, and collaborating with interdisciplinary teams to optimize patient outcomes.

Basic Requirements: English Language Proficiency (per 38 U.S.C. 7403(f)). Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE) at the time of program completion.
