The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse provides competent, evidence-based care to Veterans with complex needs in their homes.
Responsibilities include health promotion, health prevention, geriatric care, primary care, and palliative care.
The nurse delivers direct patient care and care coordination within the HBPC Program, overseeing licensed vocational nurses and nursing assistants.
Key duties involve managing complex patient situations, developing holistic care strategies, and demonstrating leadership in improving care delivery through collaborative approaches.

Education: Graduate of a professional nursing program approved by the appropriate accrediting agency (e.g., ACEN or CCNE).
