Registered Nurse - Outpatient Patient Aligned Care Team - Portland

The Registered Nurse in the Outpatient Patient Aligned Care Team (PACT) provides direct patient care in a community-based outpatient clinic setting. Responsibilities include delivering fundamental, knowledge-based care to assigned patients while developing technical competencies. The nurse integrates biopsychosocial concepts, cognitive skills, and technically competent practice to care for patients with basic or complex needs. They demonstrate proficiency in practice through conscious and deliberate planning, and are self-directed in goal setting for managing complex patient situations.
