Clinical Pharmacist Practioner, Patient Aligned Care Team

This position provides clinical pharmacy support to outpatient Patient Aligned Care Teams (PACTs) within the Ambulatory Care Service at the Robley Rex VA Medical Center. The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice, working independently under their scope of practice to directly care for patients.

United States Citizenship required. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
