Registered Nurse - Specialty Care Nurse Coordinator

The Registered Nurse - Specialty Care Nurse Coordinator is responsible for coordinating and managing the care of Veterans within specialty care services. This role involves delivering comprehensive, patient-centered nursing care, ensuring continuity and quality across the care continuum. Key responsibilities include assessing patient needs, developing and implementing care plans, and collaborating with interdisciplinary teams to optimize health outcomes.

Qualifications: Must be proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
