As a Registered Nurse in the Patient Aligned Care Team (PACT) - Outpatient Staff, you will deliver comprehensive, patient-centered primary care to Veterans in an outpatient setting. Your responsibilities include:
Direct Patient Care: Provide holistic, evidence-based nursing care to a panel of Veterans, focusing on health promotion, disease prevention, and management of acute and chronic conditions. Conduct comprehensive health assessments, develop and implement individualized care plans, and evaluate patient outcomes.
