RN- Case Manager

The Community Care Case Manager RN is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care for Veterans receiving services in the community. The role involves applying critical thinking skills to support the delivery of high-quality, timely care. Key responsibilities include coordinating care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

Basic Requirements: English Language Proficiency (per 38 U.S.C. 7403(f)). Graduate of a school of professional nursing approved by the appropriate accrediting agency, accredited by ACEN or CCNE at the time of program completion.

Veterans Health Administration operates the Clement J. Zablocki VA Medical Center, providing healthcare services to veterans. The RN CC&ICM Initiative Coordinator oversees clinical programming and daily operations of the initiative.
