Registered Nurse -- Care Coordination and Integrated Case Management

The RN CC&ICM Initiative Coordinator is administratively responsible for the clinical programming and daily operations of the Clement J. Zablocki VA Medical Center (CVAMC) CC&ICM initiative. This role has both clinical and administrative responsibility to ensure effectiveness of services, application of latest VHA guidelines, and modification of service patterns. Key responsibilities include overseeing the coordination of care and integrated case management, ensuring that patient care is delivered in accordance with established protocols and standards.
