Physical Therapist (Home-Based Primary Care) - EDRP Approved

【岗位职责】 作为家庭基础初级保健项目的物理治疗师，您将为退伍军人提供全面的、以患者为中心的居家物理治疗服务。主要职责包括：对患者的功能性活动能力、力量、平衡和疼痛水平进行初步和持续评估，以制定个性化治疗计划；实施治疗性干预措施，如治疗性运动、手法治疗、步态训练和物理因子治疗，以提高患者的独立性和生活质量；与包括医生、护士、社会工作者和职业治疗师在内的跨学科团队紧密合作，协调护理并解决复杂的医疗和心理社会需求；教育患者及其护理人员关于家庭锻炼计划、跌倒预防策略和安全活动技巧；准确及时地在电子健康记录中记录所有患者接触情况，确保符合监管和临床标准；参与团队会议、病例讨论和质量改进计划，以提升项目有效性；管理一定数量的居家退伍军人病例，并在指定地理区域内前往其住所；调整治疗方法以适应患者的家庭环境和功能限制；根据需要为物理治疗助理和学生提供监督和指导；紧跟老年和居家物理治疗领域的循证实践，以确保最佳患者治疗效果。 【任职要求】 具备物理治疗师执业资格；拥有在家庭基础或初级保健环境中工作的经验者优先；能够独立进行患者评估和制定治疗计划；具备良好的跨学科团队协作和沟通能力；能够适应频繁出差，在指定地理区域内为居家退伍军人提供服务；熟悉电子健康记录系统及相关文档规范；掌握老年患者护理和居家环境适应方面的专业知识；具备监督和指导物理治疗助理及学生的能力；持续关注并应用循证实践以优化患者结局。

To qualify, you must be a United States citizen. You must meet one of the following education and experience combinations: (1) a Bachelor's degree in Physical Therapy and five years of progressively independent experience as a physical therapist; (2) a Master's degree in Physical Therapy and two years of progressively independent experience as a physical therapist; or (3) a Doctorate degree in Physical Therapy. You must be a graduate of a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). Graduates of foreign physical therapy programs must hold a full, unrestricted, and current license to practice physical therapy in a U.S. state, territory, commonwealth, or the District of Columbia. You must hold a full, current, and unrestricted license to practice physical therapy in a U.S. state, territory, commonwealth, or the District of Columbia. You must be proficient in written and spoken English. You must meet physical requirements necessary for the role, including the ability to travel to patients' homes and perform physical therapy tasks.
