Physical Therapist (Home-Based Primary Care) - EDRP Approved

As a Physical Therapist in the Home-Based Primary Care program, you will provide comprehensive, patient-centered physical therapy services to veterans in their homes. Your primary responsibilities include conducting initial and ongoing assessments of patients' functional mobility, strength, balance, and pain levels to develop individualized treatment plans. You will implement therapeutic interventions such as therapeutic exercises, manual therapy, gait training, and modalities to improve patients' independence and quality of life. You will collaborate closely with an interdisciplinary team including physicians, nurses, social workers, and occupational therapists to coordinate care and address complex medical and psychosocial needs. You will educate patients and their caregivers on home exercise programs, fall prevention strategies, and safe mobility techniques. You will document all patient encounters accurately and timely in the electronic health record, ensuring compliance with regulatory and clinical standards. You will participate in team meetings, case conferences, and quality improvement initiatives to enhance program effectiveness. You will also manage a caseload of homebound veterans, traveling to their residences within a designated geographic area. You will adapt treatment approaches to accommodate patients' home environments and functional limitations. You will provide supervision and guidance to physical therapy assistants and students as needed. You will stay current with evidence-based practices in geriatric and home-based physical therapy to ensure optimal patient outcomes.

To qualify, you must be a United States citizen. You must meet one of the following education and experience combinations: (1) a Bachelor's degree in Physical Therapy and five years of progressively independent experience as a physical therapist; (2) a Master's degree in Physical Therapy and two years of progressively independent experience as a physical therapist; or (3) a Doctorate degree in Physical Therapy. You must be a graduate of a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). Graduates of foreign physical therapy programs must hold a full, unrestricted, and current license to practice physical therapy in a U.S. state, territory, commonwealth, or the District of Columbia. You must hold a full, current, and unrestricted license to practice physical therapy in a U.S. state, territory, commonwealth, or the District of Columbia. You must be proficient in written and spoken English. You must meet physical requirements necessary for the role, including the ability to travel to patients' homes and perform physical therapy tasks.
